Planning Your 2023 Enrollment Notice

Please click here to read an important notice being sent to you regarding Plan C 2023 Open Enrollment. This notice contains the information needed to assist you in choosing the appropriate Plan C coverage for January 1, 2023. Make note that the due date for open enrollment co-payments and/or coverage changes is December 15, 2022.

In addition, this notice also contains the quarterly CAPP charges for the quarter beginning April 1, 2023. 

This notice is also being physically mailed to you as well as being posted on the Funds website and Facebook page.
